Forms

What do you have to do to become a participant?

 
  1. Complete and sign the entire application.
  2. Complete and sign the Student Release form.
  3. Get an unofficial copy of your most recent high school transcript.  Those in grades 6th, 7th, or 8th need to submit a copy of your most recent report card.
  4. Get all required forms to our office immediately.
After we have processed your application, you will receive an acceptance letter.  At that time, please make an appointment to see your APAETS representative at your school or at our office.  Parents are encouraged to attend. 
 
Our Application Forms may be accessed and downloaded, then printed on your printer.
